Waiver & Release of Liability

1.    Acknowledgment of Risks
I understand that participation in wrestling involves physical activity, physical contact, and inherent risks of injury, including but not limited to sprains, strains, broken bones, concussions, or other serious injury. I acknowledge these risks and voluntarily allow my child to participate in Plainedge Wrestling activities.


2.    Medical Authorization
In the event of injury or illness, I authorize Plainedge Wrestling coaches, volunteers, or representatives to seek and obtain emergency medical treatment for my child. I agree to be financially responsible for any medical treatment provided.


3.    Assumption of Responsibility
I agree that my child’s participation is voluntary and at their own risk. I accept full responsibility for any costs related to injuries or damages that may occur as a result of participation.

 
4.    Release of Liability
In consideration of my child’s participation, I hereby release, discharge, and hold harmless Plainedge Wrestling, its coaches, volunteers, officers, affiliates, and facility providers from any and all claims, demands, or causes of action arising from participation in practices, competitions, travel, or events associated with the program.


5.    Code of Conduct
I understand that my child and I are expected to abide by Plainedge Wrestling’s Code of Conduct and Terms & Conditions. Failure to do so may result in suspension or removal from the program without refund.
